My ex-wife and I were divorced in Illinois and since then have both moved to Wisconsin. I am looking to transfer our document to the area that now has jurisdiction over us. First question, what forms do I use? Second, is this wise or can we continue to go to Illinois court for disputes?

Talk to a WI lawyer. If both of you now live in WI, you need to look at the laws of that state. IL courts would probably be very happy to cede jurisdiction
